This is a free, non-referral service. The Nurse will talk with you about your smoking habits and what options would best suit you.

The NHS Health Check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia.
Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions or have certain risk factors, will be invited (once every five years) to have a check to assess their risk of heart disease, stroke, kidney disease, diabetes and some forms of dementia and will be given support and advice to help them reduce or manage that risk.

We endeavour to complete private forms within 21 working days, dependent upon annual leave and sickness. Please ensure all forms handed into Reception are accompanied by a signed consent to release information form. Without this form we are unable to process your request.
Once your form has been handed in to reception they will be passed to Tracy Devine who processes the request. The report is logged onto the system and the most appropriate Doctor will review the form. Should the patient need to be seen Tracy will book them an appointment.
When the form is completed, we will inform you by telephone, and an invoice will be raised and payment will be required upon collection of the report.
Tracy works Tuesday, Wednesday, Thursday & Friday 9am – 1:30pm and can be contacted on 01584 810343.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse. The countries and areas within these countries that you are visiting determine what vaccinations are required. Some travel vaccinations may incur a charge (please see Private Fees).

It is important to make this initial appointment to determine which vaccine is required as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge above the normal prescription rate as not all travel vaccinations are included in the services provided by the NHS.
