People in England who have had a full course of the COVID-19 vaccine can demonstrate their COVID-19 vaccination status for international travel.
Please note that inline with other surgeries we do not provide mask exemption certificates or certificates of vaccination.
It would also help us greatly if you postponed ringing the surgery to enquire as to when you will receive your vaccination, you will be contacted either by letter or phone advising you when to attend as soon as we possibly can.
Please note that the practice does not hold these vaccines on site as these are being given centrally.
You will be notified by post, text or phone call. Please ensure that our surgery number is added a trusted number if you have a call screening service.

Our telephone lines are open as normal for all queries and we are still seeing patients subject to them being triaged first. Patients who just turn up at the surgery will not be seen.
We are taking 10 times the number of phone calls than normal and our staff are trying to answer the phones as quickly as possible.
Pre Vaccine Questionnairre.. Please complete this and bring to the clinic with you. If you are unable to print this off there will be some at the clinics.

Dear Patient,
Re: COVID-19 and Requests for Med3 (Fit Notes/Sick notes)
In the majority of cases, you do not need to ask your GP for a Fit Note or ‘Sick note’ for periods of absence due to COVID-19 Coronavirus.
- A high temperature
- A new, continuous cough
- A loss of, or change to, your sense of smell or taste
This is taken directly from the national guidance in relation to COVID-19 only.
www.nhs.uk/conditions/coronavirus-covid-19/self-isolation-and-treatment/when-to-self-isolate-and-what-to-do/
Employers may require fit notes for non-COVID-19 health conditions. NHS England has asked that employers exercise discretion in asking for medical evidence to support periods of sickness absence at this time.
Guidance on Specific Circumstances
Overleaf is a breakdown of potential requests for fit notes, advice, and outcomes. You might find it helpful to give this letter to your employer to help them to determine the right course of action for you.
