Are you looking after or providing support for a relative friend or neighbour? Are you being helped or supported by a relative friend or neighbour?
Your GP needs to know so you can be offered the right information, support and access to services. GP surgeries have to compile information for the government about the numbers of carers attached to the surgery so your information will help them to do this.

The Worcestershire Association of Carers is a registered charity providing an independent source of information and support for all adult carers across Worcestershire. The practice can signpost you to access the help and support the organisation offers to carers. There are leaflets and forms available at the surgery or you can simply tell the doctor or nurse that you are a carer and we will help you to complete the simple form that will put you in touch with the association. Alternatively, you can visit their website at www.carersworcs.org.uk.
If you are a Shropshire or Herefordshire patient the association will happily let your local county association know that you are a carer and need access to their support.
