New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Catchment Area

The form takes up to 10 minutes to complete and can be completed from any computer or mobile device. We’ll ask you about your personal details, medical history, and other information which helps us provide safe and efficient care to you.

Once you have completed the form, we will confirm that you have been registered successfully via text/email, or we will contact you if we need anything else.

Non-urgent advice: Online Registration

By registering online, you will usually be registered on the same day (except on Bank Holidays or at weekends) but sometimes registrations can take up to 48 hours to process. You will receive regular text and email communication about the progress of your registration.

If you require any support to register at Tenbury Surgery or Brunswick House Clee Hill, please contact us or visit the Practice.

Temporary patients

If you are not registered with this practice but need treatment whilst staying in the area, you will need to complete a temporary registration form available from the surgery.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
